In skin lesions, dupilumab decreases the mRNA expression of genes linked to TH2-inducing chemokines, inflammatory pathways, T cell activation, DCs, and eosinophil activation. While the levels of IL-17A and IL-22 did not considerably decrease, the expression of genes associated to IL-17, such as elastin (PI3), IL23p19/IL23A, and S100A8, as well as inhibitory trends (CXCL1 and S100A7), did dramatically decrease in response to 300 mg of dupilumab [1]. Utilizing microarrays and qRT-PCR, it was possible to see a significant reduction in S100A gene expression and a major inhibition of hyperplasia-related genes (such as K16) with 300 mg of dupilumab. Measurements were also made of other important epidermal changes in skin affected by severe atopic dermatitis (AD), such as notable increases in claudin and lipid product levels with 300 mg of dupilumab. As demonstrated by the reduction in K16 with Dupilumab, a dose-dependent increase in differentiation gene (LOR and FLG) expression was seen following modification to prevent epidermal hyperplasia [1].
